By booking an appointment with Milly van Dam Naturopathy, you acknowledge that you have read and agree to these terms and conditions.
Purpose of consultation
Milly van Dam Naturopathy will use the information you provide to offer personalised recommendations regarding dietary and lifestyle changes, as well as nutritional and/or herbal supplements that may support your wellbeing.
By using these services, you confirm that the information you have provided is true and accurate to the best of your knowledge and agree to advise your practitioner of any changes to your health status, medications, or medical care.
Scope of practice
Milly van Dam Naturopathy is passionate about supporting the overall health and well-being of each individual. The clinic offers naturopathic and holistic health services, focusing on supportive education and empowering clients to make informed lifestyle and nutritional choices that promote overall wellness.
These services are not a substitute for medical diagnosis or treatment. Practitioners at Milly van Dam Naturopathy do not diagnose, treat, or claim to cure any medical condition or disease.
Clients are encouraged to continue regular consultations with their medical doctor or other qualified healthcare professionals for the management of any ongoing medical conditions. Prescription medications should not be altered or discontinued without first consulting the prescribing doctor.
Payment terms
Payment is required before or at the time of your appointment, as well as before receiving any nutritional or herbal supplements. No products or services will be provided without payment, except where an alternative payment schedule has been agreed upon in advance.
If payment is not received by the due date, and no payment arrangement has been agreed upon, Milly van Dam Naturopathy reserves the right to engage a debt collection agency to recover the outstanding amount. Any additional costs incurred in the recovery of the debt (including agency fees) will be added to the total amount owing.
We kindly request that you provide at least 24 hours’ notice if you need to cancel or reschedule your appointment – to do so, please follow the link in your confirmation email.
If you cancel or reschedule within 24 hours of your scheduled appointment, a cancellation fee will apply (as stated in your confirmation email). We understand that unforeseen circumstances can arise, and exceptions may be made on a case-by-case basis. Thank you for your understanding.
If, for any reason, Milly van Dam Naturopathy needs to reschedule your appointment, as much notice as possible will be given. You will not be charged a cancellation fee, and your appointment will be rescheduled to a new time that works for you.
Cancellation & rescheduling policy
At Milly van Dam Naturopathy, your privacy is important. All personal and health information you provide is securely stored and managed through Simple Clinic, a trusted practice management platform that complies with the New Zealand Privacy Act 2020.
All consultations, whether conducted in-person or online, are treated with the same level of privacy and confidentiality.
Some personal and health information may be processed by trusted third-party service providers that help us deliver our services (such as payment processing, scheduling, and secure communications). All third-party providers are bound by privacy and security requirements and only have access to the information necessary to provide their services.
